Han Jisheng, male, Han nationality, is a native of Xiaoshan, Zhejiang Province. He graduated from the Shanghai Medical Institute. Han is a professor at the Medical Department of Beijing University, and he serves as director of the Nerve Science Institute. Han has had a distinguished career in the study of acupuncture and its effects on the body and mind. He has devoted himself to the study of the acupuncture analgesia mechanism since 1965. He was the first to describe the time-space distribution pattern of analgesia due to an acupunctured acupoint on the human body. Han also proved that the secretion of abirritate chemicals by the nervous system, including hydroxyl tryptamine and endorphin, is stimulated by acupuncture. He found that specific NPY may be released in the human brain by stimulating acupoints with electricity, when the frequency is carefully adjusted. However, he also found that if the stimulation lasts for more than 2 hours, anti-analgesia chemicals, such as CCK, would be secreted in the brain. He proposed that the effect of acupuncture relies on the amounts of analgesia and anti-analgesia chemicals in addition to the relative balance between both kinds of chemicals. A nerve stimulator developed based on the principle delivers the effect of analgesia and anti-spasm, and may be used to cure the diacetylmorphine habituation. Han Jisheng was elected an academician of the Chinese Academy of Sciences in 1993.

The monoamine hypothesis for affective disorders indicates a functional impairment of the monoamine systems in NS as the causative factor for the development of depression. Pharmacological manipulations of the monoaminergic neuronal system using tricyclics and monoamine oxidase inhibitors produced promising therapeutic effects as well as certain unwanted side effects which urged the search of a physiological means to activate the central monoamine systems. The analgesic effect produced by electroacupuncture (EA) stimulation in the rat was dose-dependently antagonized by cholecystokinin octapeptide (CCK-8) administered intracerebroventricularly (i.c.v.) or intrathecally (i.th) at a dose range of 0. 25-4 ng. This effect had an immediate onset and lasted for at least 4 h. CCK-8 per se, however, did not affect baseline tail flick latency. Rats subjected to prolonged EA stimulation developed EA tolerance as well as cross-tolerance to morphine. These tolerances could be postponed or reversed by i.c.v. or i.th injection of antiserum against CCK-8. While CCK-8 antagonized opioid analgesia, it did not affect analgesia induced by 5-hydroxytryptamine (5-HT) or norepinephrine (NE). Moreover. CCK-8 antiserum did not alter the basic level of nociception, nor did it potentiate EA analgesia in naive rats. It is concluded that prolonged EA stimulation results in a profound release of opioids which may trigger the release of CCK-8 in the central nervous system to counteract the opioid component Of EA analgesia.

In the present review some classical neurotransmitters and some neuropeptides in the CNS for mediating AA have been discussed. Some of them play a facilitatory role, while others are antagonistic. The overall effect of AA thus depends on a dynamic balance between the opposing factors at different levels of the CNS. Various neuronal pathways in the CNS are involved in transmitting the signals triggered by the acupuncture stimulation and in activating the endogenous analgesia system for suppressing nociceptive pathways . A descending inhibitory control from the PAG to the spinal cord dorsal horn neurons has already been well-defined . Recently, a neuronal circuitry in the brain has been characterized which is Very important for mediating AA and morphine analgesia. It is a neuronal loop involving the PAG, nucleus accumbens, amygdala, and habenula, as well as the arcuate nucleus of the hypothalamus. It has been tentatively termed "the meso-limbic loop of analgesia", details of which have been described else- where and summarized in a review article which will appear in the Advances in Pain Research and Therapy series. The scope of the present article is limited to the central neurotransmitters in AA. However, the implication of intracellular events such as calcium and cyclic nucleotides in AA and EA tolerance are certainly of great importance. For more detailed information the readers are referred to a monograph published in English covering most of the studies from this laboratory in the field of pain research.

Systemic studies performed in our laboratory have led to the conclusion that low frequency (2Hz) electroacupuncture (EA) accelerated the release of enkephalin in the spinal cord, whereas high frequency (100Hz) EA increased the release of dynorphin. This conclusion which was mainly based on experiments on rats has recently been confirmed in humans. The aim of the present study was to find out brain areas (centers) which would receive low- or high- frequency inputs and to transfer the relevant signals to neurons containing enkephalin or dynorphin respectively. Experiments in rats revealed that ablation of the telencephalon and diencephalon, or selective lesioning of the arcuate nucleus of hypothalamus (ARH) abolished low frequency EA analgesia (LEAA) but not high frequency EAA (HEAA). On the contrary, electrolytic or kainate lesion of the PBN of the pons led to serious attenuation of HEAA without affecting LEAA. In animals subjected to kainate lesion of the ventral periaqueductal gray (VPAG), neither LEAA nor HEAA was functioning. However, microinjection of beta-endorphin (beta-EP) antiserum into vPAG only abolished LEAA but not HEAA. In addition, strong analgesia could be elicited by direct electrical simulation of either ARH or PBN, the optimal frequencies being 1:8, respectively. In rats made tolerant to LEAA by 6hr continuous EA stimulation, there was a cross tolerance toward ARH stimulation induced analgesia, and vice versa. Cross tolerance was also existing between HEAA and PBN stimulation produced analgesia. These results indicate that: (l) Signals induced by 2Hz EA reach ARH to activate beta-EP neurons which innervate vPAG whereby descending pathway originated to release enkephalins in the spinal cord. (2) High frequency signals arrive PBN which innervate vPAG whereby descending pathway originated to release dynorphins

The effectiveness of electroacupuncture (EA) stimulation in suppressing the morphine abstinence syndrome was studied. Male Wistar rats were made dependent on morphine either by multiple injections or pellet implantation. EA of 2 Hz or 100 Hs was administered for 30 min followed by naloxone challenge (0.5 mg/kg, i.p.) and the withdrawal sydrome was scored for a period of 45 min. In rats receiving multiple injection regime, 100-HZ EA produced a statistically significant suppression of wet shakes (-61 %), teech chattering (-59 %), escape attempts (-48 %), weight loss (-3,3 %) and penile licking (-28 %) (p<0.05). EA of 2 Hz produced only a mild but significant suppression in escape attempts (-42 %) and wet shakes (-31 %). Similar results were obtained in rats receiving pellet implantation. Thirty-two patients with spinally originated muscle spasticity were treated with a transcutaneous electrical nerve stimulator, the Han's acupoint nerve stimulator (HANS) via skin electrodes placed over the acupoints on the hand and leg. High frequency (100 Hz), but not the low frequency (2 Hz), stimulation was effective in ameliorating muscle spasticity. While the therapeutic effect lasted for only 10 minutes in the first treatment, it became consolidated after consecutive daily treatment for 3 months. The anti-spastic effect induced by high frequency electrical stimulation can be partially reversed by a high dose of naloxone. The results suggest that the antispastic effect elicited by peripheral electrical stimulation is mediated, at least in part, by the endogenous opioid ligand interacting with the kappa opiate receptors, most probably dynorphin, in the central nervous system.

A special type of TENS device, the Han's Acupoint Nerve Stimulator (HANS) was used to treat 212 heroin addicts (161 male, 51 female, aged 15-38) subjected to abrupt abstinence, 30 min per day for 10 days. Our previous studies have shown that analgesia induced by electroacupuncture (EA) of different frequencies is mediated by different kinds of Opioid peptides. Thus, low frequency (2 Hz) EA increases the release of enkephalins (ENK) and beta-endorphin (END) in the central nervous system, whereas high frequency EA (100 Hz) increases the release of dynorphin (DYN) in the spinal cord. In recent years, new experiments have yielded new data that further substantiate the hypothesis put forward several years ago. 1. How to accelerate the release of endomorphin (EM) in the central nervous system EM is an produced morphine-like peptide composed of 4 amino acids, showing a high specificity to mu Opioid receptor. Han et al (1999) have shown that intrathecal injection of antiEM antibody to the rat spinal cord blocked 2 Hz but not 100 Hz EA induced analgesia. Likewise, intracerebroventricular (ICV) injection of anti-EM antibody or the mu receptor blocker CTOP attenuated 2 Hz but not 100 Hz EA-induced analgesia (Huang et al, 2000). The results suggest that 2 Hz EA accelerated the release of EM in brain and spinal cord to interact with mu Opioid receptor to produce analgesic effect. High frequency EA was not effective. 2. How to activate both neural pathways mediating both low and high frequency EA effect Pharmacological studies have shown that intrathecal injection of both encephalin and dynorphin produces a synergistic analgesic effect. It would naturally lead to the supposition that EA analgesia may be potentiated if one could activate both pathways simultaneously. Two paradigms could be used. Paradigm A is to use low frequency (LF) and high frequency (HF) alternatively, each lasting for a certain period (already optimized to be 3 sec). Paradigm B is to use LF in one limb and HF in another limb. Pharmacological studies were used to analyze the receptor mechanisms; neurochemical detection and antibody microinjection technique were used to identify the neuropeptides responsible for producing the analgesic effect. All three lines of evidence pointed to the conclusion that it was the paradigm A which produced the maximal (synergistic) effect, whereas paradigm B produced an effect almost identical to that of 100 Hz EA. The results suggested that in the later case, the signal of 2 Hz was immersed into and masked by that of 100 Hz, and could not stand out as an independent component of LF EA (to be published). 3. Effect of EA on neuropathic pain Neuropathic pain model was constructed by L5/L6 nerve ligation in the rat. Mechanical allodynia was shown by the 50 % withdrawal threshold, whereas cold-induced ongoing pain was detected by the number of paw lifts from the 5 IC cold plate in 5 min. While EA of both frequencies could reduce mechanical allodynia, the effect of 2 Hz was much stronger, with a shorter latency and longer duration as compared to that of 100 Hz stimulation. In terms of relieving the cold-induced ongoing pain (hyperalgesia), the effect of 2 Hz was also significantly stronger and longer lasting (up to 48 hr for one treatment) as compared to 2-4 hr in 100 Hz stimulation (Sun et al, to be published). 4. EA treatment for morphine withdrawal syndrome and for craving For the treatment of withdrawal syndrome, including tachycardia, weight loss and wet dog shakes in the rat, 100 Hz EA was better than 2 Hz EA. This was true not only in the rat experiment but also in the humans. High rate of relapse characterized the outcome of drug abusers after the successful detoxification, and craving may constitute the main cause of relapse. In order to study the mechanisms of Opioid craving, an animal model is indispensable. A conditioned place preference (CPP) model was constructed in this laboratory, and stress or drug priming was used for the reinstatement of the extinguished CPP. In this model, it was only 2 Hz or 2/100 Hz, but not pure 100 Hz peripheral stimulation, that was effective to suppress the CPP. Thus, a sharp contrast can be made between the optimal frequency for EA to suppress withdrawal (100 Hz better than 2 Hz) versus that for suppressing the Opioid CPP (2 Hz better than 100 Hz), suggesting a difference in the mechanisms underlying withdrawal and craving. The findings obtained in rats have to be tested in humans for the feasibility of using in addicts. A study was thus made to clarify this question. 100 Hz; stimulation was ineffective in decreasing craving, whereas the score of craving were dramatically decreased by 2 Hz or 2/100 Hz peripheral stimulation (HANS), implying an accelerated reduction of the degree of craving by HANS containing a low frequency component. To summarize, among various factors affecting the efficacy of electroacupuncture for medical use, the frequency of stimulation seems to be of crucial importance. While both classical, neurotransmitters and neuropeptides in the central nervous system can be activated by electroacupuncture, the neuropeptides are most sensitive to the frequency of stimulation The information discussed in this article may have theoretical implication in neuroscience research, as well as practical implication in clinical practice. 5. Brain areas in humans involved in low frequency EA-induced analgesia. High rate of relapse to drug-using behavior after long period of abstinence characterizes the behavior of experienced users of heroin and other drugs of abuse, and the relapse remains the primary problem for treatment. In the present study we built a putative animal model that mimic human relapse i. e., the reinstatement of morphine-induced conditioned place preference (CPP) in rats. In this study, we found electroacupuncture (EA) with low frequency (2 and 2/100 Hz) could inhibit drug priming or footshock-induced CPP reinstatement in rats when it was given 18 hours before reinstatement, and these effects were found to be naloxone reversible, suggesting a mechanism involving the activation of opioid receptors by endogenous opioid ligands; while EA with high frequency (100 Hz) had no effect. Previous studies in our lab have amply shown that low frequency (2 Hz) stimulation could increase the release of enkephalin which acts on u and o opioid receptors up the spinal level, while high frequency (100 Hz) stimulation could increase the release of dynorphin which interacts with k opioid receptor at spinal level. So we concluded the effect of EA with low frequency on relapse involving a mechanism of the activation of opioid receptors by endogenous opioid ligands above the spinal level. And we suggest that EA may be used as a putative measure for the prevention of relapse to drug use in humans.

It was reported that acupuncture or electro-acupuncture (EA) is effective in reducing the body weight for obese patients, although the mechanisms remain obscure. In a previous study, we have found that rats fed with high-fat (HIF) diet developed diet-induced obesity (DIO) with a concomitant decrease in the hypothalamic content of the cocaine and amphetamine-regulated transcript (CART) peptide, a peptide with anorexiogenic effect. To assess the central effect of EA on DIO rat, we revealed that EA up-regulated the expression of CART peptide in the arcuate nucleus (ARC) of the DIO rats. After feeding with HIF diet for 14 weeks, the DIO rats received EA stimulation three times per week for 4 weeks. The expression of CART peptide in ARC was measured using immunohistochemistry. The plasma ACTH was measured with ELISA. EA caused a reduction of both body weight and energy intake in DIO rats and increased the expression of CART peptide in ARC. The plasma ACTH was increased in response to restraint stress, but EA produced no further increase in ACTH levels. The results suggest that EA can up-regulate the expression of CART peptide to approach normal level, resulting in an inhibition of food intake and a reduction

Previous studies suggested that electroacupuncture (EA) can suppress opioid dependence by the release of endogenous opioid peptides. To explore the site of action and the receptors involved, we tried to inject highly specific agonists for mu-, delta- and kappa-opioid receptors into the CNS to test whether it can suppress morphine- induced conditioned place preference (CPP) in the rat. Male Sprague-Dawley rats were trained with 4mg/kg morphine, i.p. for 4 days to establish the CPP model. This CPP can be prevented by (a) i.p. injection of 3mg/kg dose of morphine, (b) intracerebroventricular (i.c.v.) injection of micrograms doses of the selective mu-opioid receptor agonist DAMGO, delta-agonist DPDPE or kappa-agonist U-50,488H or (c) microinjection of DAMGO, DPDPE or U50488H into the shell of the nucleus accumbens (NAc). Electroacupuncture (EA) has been reported to reduce body weight in overweight subjects in clinical practice, as well as in rats and mice with diet-induced obesity. In the present study, this effect of EA was tested in lean rats subjected to long-term food restriction (FR, food was offered only 1h/day). Two hertz EA administered once every other day produced a further reduction in body weight in FR rats. Exploration of the mechanism involved revealed significant downregulation of the orexigenic peptides: ghrelin in the stomach, and neuropeptide Y (NPY) but not Agouti-related peptide (AgRP) in the hypothalamus, which was in line with the reduction in food intake in rats receiving EA stimulation as compared with those receiving restraint only. Uncoupling protein 3 (UCP3), involved in accelerating energy expenditure, was not significantly altered. These results suggest that the EA-induced body weight reduction was due mainly to a decrease in food intake rather than an increase in energy expenditure. A reduction in the orexigenic peptides ghrelin and NPY may be involved in the underlying mechanism.

cholecystokinin octapeptide (cck-8) is a physiological antagonist of endogenous opioids in the central nervous system (cns). our previous work has shown that cck-8 plays an important role in the development of tolerance to morphine analgesia and electroacupuncture (ea) analgesia in the rat. the present studies were designed to examine whether the cck(b) receptor is involved in the modulation of ea analgesia and the development of ea tolerance in mice. the latency to flick the tail in the radiant heat was used as index to assess the efficacy of ea analgesia. subcutaneous (s.c.) injection of the cck(b) receptor antagonist l365,260 produced a dose-dependent (0.125-2.0mg/kg) potentiation of the analgesia induced by 100hz ea, with a maximal effect occurred at 0.5mg/kg. in addition, l365,260 (0.5mg/kg) significantly reversed chronic tolerance to 100hz ea in mice. these results suggest that the cck(b) receptor might play a role in the tonic inhibition of 100hz ea-induced analgesia and in the mediation of chronic tolerance to 100hz ea in mice. the results opened a way for further investigation of the

Transcutaneous electrical nerve stimulation (TENS) has been shown to be an effective measure for pain relief. The aim of the present study was to determine the optimal intensity and interval of repeated 100 Hz TENS for the treatment of chronic inflammatory hyperalgesia in a monoarthritic pain model of the rat, and to assess the changes of the spinal substance P (SP) release in response to TENS treatment. A reliable, reproducible chronic monoarthritic pain model was produced by intra-articular injection of complete Freund's adjuvant (CFA) at single ankle joint. The efficacy of 100 Hz TENS treatments with different frequencies and intensities was compared. In the acute period (within 3 weeks) of monoarthritis, twice-a-week schedule of TENS reduced the swelling of the inflamed ankle significantly. In the stable period (4-9 weeks), however, once-a-week schedule produced a significantly better therapeutic effect on both inflammation and arthritic hyperalgesia than that of twice- or five- times-a-week schedule. Using three levels of intensity of TENS, we found that the weaker (1-1-2 mA) stimulation produced significantly better therapeutic effects. Repeated TENS produced a reduction of SP content in spinal perfusate in parallel with the progressive reduction of the arthritic pain scores. Our results suggest that (i) consecutive TENS treatments produced cumulative effect for chronic hyperalgesia, (ii) for chronic inflammatory hyperalgesia, a weaker intensity and more sparsely arranged treatment schedule may produce better therapeutic effect and (iii) a decrease in SP release may serve as one of the possible neurochemical mechanisms underlying the therapeutic effects of multiple TENS treatments on chronic inflammatory hyperalgesia.

Our previous study has reported that electroacupuncture (EA) at low frequency of 2 Hz had greater and more prolonged analgesic effects on mechanical allodynia and thermal hyperalgesia than that EA at high frequency of 100 Hz in rats with neuropathic pain. However, how EA at different frequencies produces distinct analgesic effects on neuropathic pain is unclear. Neuronal plastic changes in spinal cord might contribute to the development and maintenance of neuropathic pain. In the present study, we investigated changes of spinal synaptic plasticity in the development of neuropathic pain and its modulation by EA in rats with neuropathic pain. Field potentials of spinal dorsal horn neurons were recorded extracellularly in sham-operated rats and in rats with spinal nerve ligation (SNL). We found for the first time that the threshold for inducing long-term potentiation (LTP) of C-fiber-evoked potentials in dorsal horn was significantly lower in SNL rats than that in sham-operated rats. The threshold for evoking the C-fiber-evoked field potentials was also significantly lower, and the amplitude of the field potentials was higher in SNL rats as compared with those in the control rats. EA at low frequency of 2 Hz applied on acupoints ST 36 and SP 6, which was effective in treatment of neuropathic pain, induced long-term depression (LTD) of the C-fiber-evoked potentials in SNL rats. This effect could be blocked by N-methyl-d-aspartic acid (NMDA) receptor antagonist MK-801 and by opioid receptor antagonist naloxone. In contrast, EA at high frequency of 100 Hz, which was not effective in treatment of neuropathic pain, induced LTP in SNL rats but LTD in sham-operated rats. Unlike the 2 Hz EA-induced LTD in SNL rats, the 100 Hz EA-induced LTD in sham-operated rats was dependent on the endogenous GABAergic and serotonergic inhibitory system. Results from our present study suggest that (1) hyperexcitability in the spinal nociceptive synaptic transmission may occur after nerve injury, which may contribute to the development of neuropathic pain; (2) EA at low or high frequency has a different effect on modulating spinal synaptic plasticities in rats with neuropathic pain. The different modulation on spinal LTD or LTP by low- or high-frequency EA may be a potential mechanism of different analgesic effects of EA on neuropathic pain.
